By accepting and submitting this declaration you confirm that:
1) The work undertaken in servicing the above product has been done so only in full accordance with the Instructions supplied by Michell Instruments Ltd.
2) The instructions provided have been followed accurately and in full, in the order they are written and all of the instructions have been completed in their entirety. No adaptations or truncations of the provided instructions have been applied.
3) I understand and accept that any departure from, or failure to entirely comply with, the servicing instructions provided by Michell Instruments Ltd. will invalidate any warranty this product would otherwise hold.
4) The work has been carried out by suitably trained & approved personnel and the work undertaken is verified by independent inspection.
5) I understand and accept that only service items & components supplied directly by Michell Instruments Ltd. are permitted to be used in the servicing of this product. Any other independently sourced items and components that may be considered by the end user of the equipment (as detailed above) as identical and/or interchangeable will not be sanctioned for use by Michell Instruments Ltd.
6) I understand and accept that the use of any parts in the servicing of this product, other than those supplied directly by Michell Instruments Ltd., will invalidate any current warranties relating to that product.
7) All parts removed from the above detailed product will be suitably and responsibly disposed of in such a manner that they may never be reused again on the above detailed product or any other product.
8) I understand and accept that Michell Instruments Ltd. does not accept any responsibility for any servicing work carried out on products designed by Michell Instruments Ltd. by personnel other than those employed directly by, or operating expressly as an agent of, Michell Instruments Ltd.
9) I understand and accept that without fully completing, accepting and submitting this form within 7 days of the work being carried out, any service work that has been carried out on the above detailed product will not be registered by Michell Instruments Ltd. and any warranties that might otherwise apply will immediately become void.
